When Is Conservative Treatment Appropriate?
Not every orthopaedic problem requires surgery. In fact, a significant proportion of musculoskeletal conditions—including early-stage arthritis, soft-tissue injuries, tendinopathies, and many types of back pain—can be effectively managed without an operation. Conservative treatment is typically the first line of care, and surgery is considered only when non-operative measures have been given an adequate trial or when the clinical situation demands immediate intervention.
Medication Management
Pharmacological therapy plays an important supporting role in musculoskeletal care. Depending on the diagnosis, we may prescribe:
- Non-steroidal anti-inflammatory drugs (NSAIDs) for pain and inflammation.
- Disease-modifying agents in coordination with a rheumatologist for inflammatory arthropathies.
- Neuropathic pain medications such as gabapentinoids for nerve-related symptoms.
- Calcium and vitamin-D supplementation for patients with osteoporosis or metabolic bone disease.
- Topical analgesics for localised pain that does not warrant systemic medication.
All prescriptions are tailored to the individual, with careful attention to co-morbidities, drug interactions, and the patient's preferences.
Injection Therapies
Image-guided injections can deliver targeted relief and are often used both diagnostically and therapeutically:
- Corticosteroid injections for bursitis, tendinitis, and joint inflammation.
- Viscosupplementation (hyaluronic acid) for mild-to-moderate knee osteoarthritis.
- Platelet-rich plasma (PRP) for tendon and ligament injuries, leveraging the body's own healing factors.
- Nerve blocks for diagnostic localisation of pain sources and short-term relief.
Injections are performed under ultrasound or fluoroscopic guidance to ensure accuracy and maximise efficacy.
Bracing and Orthotics
Custom and off-the-shelf braces, splints, and orthotic insoles can provide mechanical support, correct alignment, and relieve pain. Common applications include knee unloader braces for unicompartmental arthritis, ankle-foot orthoses for drop foot, wrist splints for carpal tunnel syndrome, and custom insoles for plantar fasciitis or flat feet. Our team ensures proper fitting and patient education so that devices are used effectively.
Lifestyle and Activity Modification
Sustainable musculoskeletal health often requires changes in daily habits. We counsel patients on weight management, ergonomic adjustments at the workplace, safe exercise regimens, and activity modifications that protect vulnerable joints without sacrificing quality of life. These behavioural strategies can slow disease progression, reduce pain, and in many cases delay or eliminate the need for surgery altogether.
